Patient User Guide
Welcome to CareLog! This guide will help you navigate and use the system as a patient.
Getting Started
Logging In
- Open CareLog in your web browser
- Enter your email address
- Enter your password
- Click Login
TIP
Your email address is your username. If you forget your password, contact your hospital administrator.
Patient Dashboard
After logging in, you'll see your dashboard with quick access to:
- Your recent health logs
- Upcoming appointments
- Recent notifications
- Emergency call button
- Quick actions
Daily Health Logging
Track your health status daily to help your care team monitor your wellbeing.
How to Log Your Health
Navigate to Daily Health Logging from the sidebar
Click Add New Health Log
Fill in the information:
- Physical Status: Select all that apply (e.g., fever, pain, fatigue)
- Emotional Status: How you're feeling emotionally
- Symptoms: Describe any symptoms you're experiencing
- Sensitive: Toggle if this information should be private
Click Submit
WARNING
Sensitive Data: When marked sensitive, this information will only be visible to you and medical staff (not family members).
Viewing Your Health History
- All your health logs are displayed in chronological order
- Filter by date or search for specific entries
- View trends and patterns in your health data
Requesting Appointments
Schedule appointments with your healthcare providers.
How to Request an Appointment
- Navigate to Appointments & Requests from the sidebar
- Click Request New Appointment
- Fill in the details:
- Reason: Brief description of why you need the appointment
- Preferred Date/Time: When you'd like to be seen
- Priority: Normal or Urgent
- Click Submit Request
Appointment Statuses
- Pending: Waiting for doctor assignment
- Scheduled: Confirmed with date and time
- Completed: Appointment has taken place
- Cancelled: Appointment was cancelled

- View all your appointments in one place
- See who your appointment is with
- Check appointment dates and times
- View appointment notes and outcomes
Viewing Your Medical Records
Access your complete medical history.
What's Included
- Diagnoses: All conditions diagnosed by your doctors
- Prescriptions: Current and past medications
- Appointments: Complete appointment history
- Health Logs: Your daily health tracking data
How to Access
- Navigate to My Medical Records from the sidebar
- Browse through different sections:
- Diagnoses: View conditions and treatment notes
- Prescriptions: See medications, dosages, and instructions
- Timeline: Chronological view of your medical history
INFO
Medical records are updated by your healthcare providers. You cannot edit these directly.
Emergency Calls
In case of a medical emergency, use the emergency call feature.
When to Use Emergency Calls
- Severe pain or discomfort
- Sudden changes in condition
- Urgent medical concerns
- Need immediate medical attention
DANGER
For Life-Threatening Emergencies: Call your local emergency services (911, 999, etc.) immediately!
How to Make an Emergency Call
- Click the Emergency Call button on your dashboard
- Or navigate to Emergency from the sidebar
- Select the urgency level:
- CRITICAL: Immediate life-threatening situation
- HIGH: Urgent attention needed
- MEDIUM: Needs attention soon
- Describe the emergency
- Click Submit Emergency Call
After Submitting
- A nurse will be notified immediately
- You'll see the call status update
- Emergency calls can be escalated if needed
- You'll be notified when someone responds
Managing Visit Requests
Allow family members to visit you at the hospital.
How to View Visit Requests
- Navigate to Family Visits from the sidebar
- See all visit requests from family members
- Review details:
- Who is requesting to visit
- Proposed visit date and time
- Visit purpose or message
Approving Visits
- Click Approve to allow the visit
- Click Deny if you don't want the visit
- Add notes if needed

Stay informed about your care.
Types of Notifications
- Appointment confirmations and reminders
- Test results available
- Prescription updates
- Visit request updates
- System announcements
Managing Notifications
- Click the notification icon in the header
- View unread notifications
- Click on a notification to read details
- Mark as read when reviewed

- Navigate to Feedback from the sidebar
- Select the category:

- General Feedback
- Write your detailed feedback
- Click Submit
Viewing Your Feedback
- See all your submitted feedback
- Track response status
- View administrator responses
Privacy and Security
Your Data Privacy
- Your health information is protected
- Only authorized medical staff can view your records
- You control what family members can see (via sensitive flag)
- All access is logged for security
Marking Information as Sensitive
When logging health data:
- Toggle the "Sensitive" option
- Sensitive logs are hidden from family members
- Medical staff can still access all information for your care
Account Security
- Use a strong, unique password
- Don't share your login credentials
- Log out when finished, especially on shared devices
- Report any suspicious activity to your administrator

- Be Detailed: Provide thorough descriptions of symptoms
- Check Regularly: Review notifications and appointments frequently
- Use Appropriately: Reserve emergency calls for genuine emergencies

Troubleshooting
Can't Log In
- Verify your email address is correct
- Check your password (case-sensitive)
- Contact your administrator if you need a password reset
Not Seeing Expected Information
- Ensure you're logged in with the correct account
- Check if your account has been disabled
- Contact support if problems persist
Technical Issues
- Try refreshing your browser
- Clear your browser cache
- Try a different browser
- Submit feedback describing the issue
